Job summary

Fallon Health is seeking a Rehabilitation Manager for the Summit ElderCare program in the Worcester area. You will oversee the rehabilitation department, coordinate services, and work with the interdisciplinary team to meet participant needs in a home- and community-based setting.

Ideal candidates are licensed in Physical or Occupational Therapy in Massachusetts, with leadership experience, and CPR certification or willingness to obtain.

Qualifications

• Graduate of an accredited Physical or Occupational Therapy program.
• Licensed as a physical or occupational therapist in Massachusetts.
• CPR certification or willingness to obtain.

Responsibilities

• Oversee the Rehabilitation Department for the site and coordinate care with FH and SE policies.
• Ensure assessments are completed and documented to identify rehabilitation needs.
• Collaborate with the IDT, participants, and caregivers to meet rehabilitation needs.
• Lead staffing, training, and performance evaluations for rehabilitation personnel.
• Ensure timely care plans, documentation, and communication of revisions.
