Job summary

Fallon Health in Massachusetts is seeking a site-level Rehabilitation Department Manager for Summit ElderCare (PACE). This role oversees rehabilitation services, drives care planning with the interdisciplinary team, and provides direct care when needed to ensure high-quality participant outcomes.

Responsibilities include staffing, supervision, documentation in the EMR, coordination with contracted vendors, and adherence to policies.

Qualifications

• Graduate of an accredited program in Physical or Occupational Therapy.
• Licensed as a physical or occupational therapist in Massachusetts.
• CPR certification or willingness to obtain.

Responsibilities

• Oversee the Rehabilitation Department for the site and ensure program compliance.
• Coordinate rehabilitation services and care planning with the IDT.
• Assess participant needs and supervise staff in home and center settings.
• Manage staff scheduling, performance evaluations and staff development.
• Ensure timely documentation in the EMR and accurate claims support.
• Collaborate with Home Care Manager on home safety and equipment needs.
