Job summary

Fallon Health seeks a dynamic Rehabilitation Director to oversee the site Rehabilitation Department. You will ensure compliant, high-quality rehab services for Summit ElderCare participants, coordinating with the IDT and contracted vendors as needed.

The role requires directing staff, conducting assessments, care planning, and maintaining EMR documentation, while driving program efficiency and participant safety across home and center-based care.

Qualifications

• Graduate of an accredited program in Physical or Occupational Therapy.
• Massachusetts licensure as a physical or occupational therapist.
• CPR certification or willingness to obtain.

Responsibilities

• Oversee Rehabilitation Department at the site to ensure compliant, high-quality care.
• Coordinate rehabilitation services and collaborate with IDT to meet participant needs.
• Ensure timely assessments, care planning, and documentation in the EMR.
• Delegate workload to SE rehab staff, schedule staffing, and supervise performance.
• Support home-based assessments and equipment needs, ensuring cost management.
