Job summary

Fallon Health is seeking a Registered Nurse to provide direct nursing care to Summit ElderCare (PACE) participants in multiple settings. You will perform comprehensive nursing assessments, participate in care planning, and document all care and changes accurately.

Qualified candidates hold an MA RN license, CPR and Alzheimer's certification (or willingness to obtain within 60 days), with at least two years of direct adult or elderly care experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current license as an RN in the state of Massachusetts.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days of hire.
• At least two years of recent direct care experience for adults or chronically disabled individuals, with elder care preferred.

Responsibilities

• Provide direct nursing care to PACE participants in any setting following nursing process.
• Conduct comprehensive nursing assessments and participate in care planning.
• Document care and changes in health status in the medical record.
• Collaborate with the interdisciplinary team to meet participant needs.
