Job summary

Brooks Rehabilitation is seeking a Case Manager to lead discharge planning for inpatients, coordinating with a multidisciplinary team and families to ensure successful community reentry.

The ideal candidate will have RN or social work credentials and at least 1 year of acute care or inpatient rehab experience, with strong communication and leadership skills to manage a 12-person Case Management team.

Qualifications

• RN or MSW/LCSW/CRC with CCM certification highly preferred.
• 1 year experience as Case Manager or Social Worker in Acute Care or Inpatient Rehab.
• Experience with medically complex populations and coordinating with families.

Responsibilities

• Serve as facilitator for patient care teams to ensure smooth discharge planning.
• Develop creative discharge plans to support length of stay goals in inpatient rehab.
• Coordinate with insurance, community resources, and hospital staff to optimize patient outcomes.
