Brooks Rehabilitation is recruiting a Case Manager to lead patient-centered case management in an inpatient rehabilitation setting. You will coordinate with physicians, nursing, social work, and therapy teams to ensure smooth transitions and effective discharge planning.

The role requires knowledge of local resources, strong communication, and the ability to manage complex cases with a 12-person Case Management team. A Bachelor's degree and clinical background are required.

Qualifications

• RN, MSW, LCSW, CRC, CCM with Bachelor's degree as a minimum.
• 1 year experience as a Case Manager or Social Worker in Acute Care or Inpatient Rehabilitation.
• Experience with patients with acquired brain injuries or medically complex populations.
• Strong communication to facilitate discharge planning with families and teams.
• Ability to manage a caseload independently and work with a team.

Responsabilités

• Facilitate the case and the team working with the patient to support positive outcomes.
• Develop creative discharge plans to support patients in inpatient rehabilitation.
• Apply utilization management criteria and coordinate with all stakeholders.
• Delegate tasks to ensure efficiency in day-to-day casework.
