• Interviews patients/families to obtain data regarding home environment, financial resources, health and social histories to address patient needs.
• Orients patients/families to the rehab process and discusses discharge planning options.
• Provides case management services including emotional support to patients/families to facilitate adjustment and post-rehab planning.
• Documents interventions and developments in the patient\'s medical record with accuracy and timeliness.
• Acts as a liaison between colleagues, departments, and community resources to coordinate discharge plans.
