Job summary

Lehigh Valley Health Network, based in the Lehigh Valley, seeks a Mental Health Social Worker to provide assessed, diagnosed, treated mental health services and crisis intervention for adults and/or children. You will coordinate care, help patients access community resources, and support discharge planning with the treatment team.

Responsibilities include managing utilization, conducting assessments, ensuring clear documentation, and acting as liaison with families, referral sources, and funders

Qualifications

• Must have Master’s degree in Social Work, Counseling, or related field.
• Ability to remain patient and calm in stressful situations.
• Knowledge of community resources and how to refer to services.
• Understanding of psychosocial/medical needs to form integrated treatment plans.
• LCSW, LICSW or PA licensure in related fields (PA).

Responsibilities

• Oversees utilization management activities including authorization and appeals.
• Ensures discharge planning documentation is clear and timely.
• Conducts clinical assessments for interpretation and treatment planning.
• Provides case management and care coordination for clients/patients.
• Acts as liaison between patients, referral sources, and funders.
• Manages mental health commitment processes as regulated by PA law.
• Participates in department quality improvement activities.
