Job summary

Lehigh Valley Health Network seeks a licensed social work professional to provide mental health assessment, diagnosis, treatment, and crisis intervention services for adults and/or children. You will coordinate with care teams and assist patients in accessing community resources to support their recovery.

The role includes case management, discharge planning, and liaison work with referrals and funders to promote community stability within PA’s regulatory framework.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• Must hold PA licensure (LCSW/LPC/LSW or eligible).
• Ability to remain patient and calm in stressful situations.
• Understanding of psychosocial/medical needs and referrals.

Responsibilities

• Oversees utilization management and authorization of services.
• Ensures discharge planning documentation aligns with treatment.
• Conducts clinical assessments with patients and families.
• Provides case management and care coordination.
• Acts as liaison between patients, referrals, and funders for stability.
• Manages involuntary commitment processes per state law.
• Participates in quality improvement activities.
