Qualifications

• Master’s Degree in Social Work or related field.
• Licensed in Pennsylvania (LCSW/LPC/LSW).
• Ability to develop integrated, comprehensive treatment plans.
• Knowledge of community resources and referral processes.
• Experience with psychosocial/medical needs assessment and case management.

Responsibilities

• Oversee utilization management and authorization for services.
• Ensure clear, timely discharge planning documentation.
• Conduct clinical assessments with patients and families.
• Provide case management and care coordination.
• Advocate for patients to achieve community stability.
• Act as liaison between patients, referrals, and funders.
• Manage involuntary and voluntary commitment processes as regulated.
• Participate in quality improvement activities.
