Job summary

Lehigh Valley Health Network is seeking a licensed mental health professional to provide assessment, diagnosis, treatment, and crisis intervention for adults and/or children with diverse needs. You will coordinate care and help patients access services in the community in the least restrictive environment.

Qualifications

• Master’s degree in social work, counseling, or related field.
• Knowledge of psychosocial/medical needs to formulate integrated treatment plans.
• LCSW, LPC, or LSW licensed in Pennsylvania.
• Ability to remain patient and calm in stressful situations.
• Knowledge of community resources and how to refer to services.

Responsibilities

• Oversees all utilization management activity including authorization of services and denial/appeal processes.
• Ensures discharge planning documentation is clear, timely, and aligned with treatment plans.
• Conducts clinical assessments for interpretation and treatment with patients and families.
• Provides case management and care coordination for clients/patients.
• Intercedes to help patients achieve community stability.
• Acts as liaison between patients, referral sources, and funders.
• Manages voluntary and involuntary commitment processes as regulated.
• Participates in quality improvement activities for the department.
