Job summary

Lehigh Valley Health Network seeks a Licensed Psychiatric Case Manager to join our Psychiatry/Rehab and Case Management team. This full‑time day shift role provides mental health assessment, diagnosis, treatment planning, and crisis intervention for adults and/or children, coordinating services to keep patients in the community.

Responsibilities include utilization management, discharge planning, care coordination, and acting as liaison between patients, referrals, and funders.

Qualifications

• Master’s degree in Social Work, Counseling, or related field.
• LCSW / LPC / LSW license in PA.
• Knowledge of community resources and referrals.
• Ability to remain calm in stressful situations.

Responsibilities

• Oversees utilization management including authorization of services and appeals.
• Ensures discharge planning documentation aligns with treatment recommendations.
• Conducts clinical assessments with patient and families.
• Provides case management and care coordination for clients/patients.
• Intercedes to help patients achieve community stability.
• Liaisons between patients, referrals and funders.
• Manages voluntary/involuntary commitments per PA MH law.
• Participates in quality improvement activities.
