Responsibilities

• Supervise the Lead Case Manager, Case Managers, and Community Health Workers to ensure program quality and compliance.
• Coordinate supplemental programs and projects with community partners.
• Participate in meetings and provide guidance based on weekly progress reports.
• Conduct/approve chart audits and ensure timely data entry in the database.
• Ensure adherence to safety protocols and program timelines.
• Travel as needed and work evenings/weekends as required.
