Job summary

Qualifications

• Responsible for designing, developing, and implementing community health initiatives.
• Seven or more years of relevant experience and leadership in cross-functional teams.
• Experience facilitating diverse community groups and partnerships.
• Strong research, written and oral communication skills.
• Knowledge of population health data interpretation and reporting.
• Experience with Electronic Health Records (EHRs) data management and reporting.

Responsibilities

• Coordinate Presbyterian community health initiatives and ensure successful implementation and sustainability.
• Lead engagement with internal and external stakeholders, including government entities and providers.
• Develop and deploy strategies to align practitioners with new community and clinical linkage models.
• Prepare program reports and grant deliverables for Presbyterian senior leaders.
• Oversee data collection, analysis, and reporting to support decision making.
• Support grant proposals and tracking of grant deliverables.
