Job summary
Friend Health in Chicago, IL seeks a Program Manager of Community-Based Services to lead ACT, CST, and FDDP programs. You will ensure clinical quality, program fidelity, and financial sustainability while overseeing staff and partnerships with community organizations.
The role requires a strong background in behavioral health, supervisory experience, and familiarity with regulatory requirements to support high-quality community-based services.
Qualifications
- Master's degree in Social Work from a CSWE-accredited program.
- Current, unrestricted Illinois LCSW license.
- Five years of behavioral health/community mental health experience preferred.
- Three years of supervisory, program management, or clinical leadership experience preferred.
- Experience with serious mental illness and intensive community-based services preferred.
- ACT, CST, FDDP or comparable program experience preferred.
- Knowledge of IM+CANS, IDHS/DMH, CCBHC, Medicaid and related requirements.
Responsibilities
- Provide clinical, administrative, and operational oversight of ACT, CST, and FDDP programs.
- Supervise program supervisors and ensure clinical quality, staff performance, and outcomes.
- Ensure services are delivered at the appropriate frequency and location based on client needs.
- Oversee referrals, census, and transitions between levels of care.
- Review IM+CANS, treatment plans, progress notes, and related documentation.
