• Active, unrestricted medical license in the state of practice.
• Current, unrestricted DEA license.

Responsibilities

• See patients in outpatient setting with a focus on preventive care and chronic disease management.
• Provide longer appointment times and comprehensive, patient-centered care.
• Collaborate with a clinical and operational team in a value-based care model.
