Humana Inc. CenterWell Senior Primary Care in Las Vegas seeks a Primary Care Physician to join a multidisciplinary team delivering personalized, high-quality care to seniors. You will work within a value-based care model with nurse practitioners, pharmacists, care coaches, and behavioral health specialists.

The role emphasizes integrated team-based care, ample patient time, and a supportive culture with strong work-life balance and opportunities for professional growth.

Qualifications

• Graduate of accredited MD or DO program.
• Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine.
• Current and unrestricted medical license or willing to obtain license in state of practice; eligible for other states as needed.
• Excellent verbal and written communication skills.
• Demonstrate strong interpersonal relationships and teamwork in integrated care.
• Willingness to learn and adapt to value-based care setting.
• Participation in TB screening program is required.

Responsabilités

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Coordinate referrals, hospital/SNF care, DME, and home health services.
• Participate in daily huddles; CAs may lead huddles.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and growth initiatives at the center.
• Ensure compliance with licensing and accreditation requirements.
