Job summary

Humana Inc. through CenterWell Senior Primary Care seeks a Primary Care Physician to lead a team-based care model for seniors. You will provide high-quality clinical care in a value-based system, collaborating with doctors, APPs, nurses, and care coordinators to deliver coordinated services and improved outcomes.

This full-time role emphasizes direct patient care with base pay plus incentive bonuses, sign-on options, and comprehensive benefits within a supportive multidisciplinary environment.

Qualifications

• MD or DO degree from an accredited program and board eligibility/certification
• ACGME/AOA residency completed or eligible for Family Medicine, Internal Medicine, or Geriatric Medicine
• Current unrestricted medical license or willingness to obtain license in state of practice
• Strong communication and collaboration within an integrated care team
• Willingness to participate in TB screening and value-based care setting

Responsibilities

• Evaluate and treat center patients per standards of care
• Coordinate patient services, referrals, hospital/SNF coordination
• Lead active participation in the care team through daily huddles
• Ensure timely medical record documentation and coding accuracy
• Maintain compliance with licensing, certification, and accrediting bodies
• Spend 100% of time on direct patient care and related administrative tasks
