Humana Inc. CenterWell Senior Primary Care seeks a Lead Primary Care Physician to drive team-based care for seniors in a value-based model. The role emphasizes coordinated care, data-driven outcomes, and a concierge patient experience.

The PCP will lead a multidisciplinary team, spend significant time in direct patient care, and participate in growth opportunities while meeting licensing and credentialing requirements.

Formación

• Current and unrestricted medical license or willingness to obtain in state of practice.
• Graduate of an MD or DO program.
• Excellent verbal and written communication skills.
• Committed to integrated team-based care and learning in a value-based setting.
• Willing to participate in TB screening program.

Responsabilidades

• Evaluates and treats center patients in line with standards of care.
• Monitors care quality using data and chart reviews.
• Coords patient services, including referrals and hospital/SNF coordination.
• Acts as a medical expert within the care team during daily huddles.
• Supports leadership in promoting cooperative practice culture.
• Completes medical records promptly with coding optimization.
• Complies with Clinical Leadership policies and protocols.
• Meets with Regional Medical Director about care quality and outcomes.
• Participates in growth opportunities and on-call programs as needed.
• Maintains licensing and certifications; adheres to professional standards.
