Job summary

Humana, Inc. is seeking a Primary Care Physician for its CenterWell Senior Primary Care team, onsite in Nashville area. You will provide outpatient care to adults and seniors, collaborate with interdisciplinary teams, and participate in quality improvement initiatives.

The role offers a team-based, patient-centered model with ample time to build relationships, a stable schedule, and a comprehensive benefits package. Board-certified family or internal medicine or geriatrics preferred.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Fellowship.
• Willing to participate in quarterly limited on-call coverage by phone.
• Proficiency with electronic health records and digital documentation.
• 2 years' experience managing high-acuity geriatric patients.
• Supervise Advanced Practice Providers (NPs) in your assigned clinic.
• Specialty training in outpatient Primary Care, Family Medicine, Internal Medicine, or Geriatrics.
• Experience in Value-Based Care settings.
• Athena Health proficiency.

Responsibilities

• Report to the Associate Medical Director for the TN market.
• Work fully onsite at CW Bellevue: Nashville, TN 37221.
• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement programs.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to strategic programs and innovations in care delivery.
• Exercise independent clinical judgment in patient management.
