Job summary

Humana Inc. is seeking a Primary Care Physician to join our CenterWell Senior Primary Care team in New Albany, IN.

You will provide comprehensive outpatient care to adult and senior patients in a collaborative, patient-centered environment. The role requires board certification in Family Medicine, Internal Medicine, or Geriatrics, strong EHR skills, leadership of Advanced Practice Providers, and commitment to value-based care.

Qualifications

• MD or DO from an accredited medical school.
• Active license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatrics.
• Willingness to participate in quarterly limited on-call coverage by phone.
• Proficiency with electronic health records and digital documentation.
• 2 years' experience managing high-acuity geriatric patients.
• Supervise Advanced Practice Providers (NPs) in your assigned clinic.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain and document accurate medical records.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement programs.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to strategic programs and innovations in care delivery.
• Exercise independent clinical judgment in patient management.
