Job summary

Heartline Staffing is recruiting a Primary Care Physician to provide preventive primary care for adults 65+ within a value-based care model. The patient population is primarily Medicare Advantage, with a focus on chronic disease management and preventive care.

Approximately 15 patients are seen per day with no RVU pressure. You will work with an integrated care team including NP/PA, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine is required.

Responsibilities

• Provide preventive primary care for adults 65+.
• Focus on chronic disease management and preventive care.
• Practice within a value-based care model.
• Collaborate with an integrated care team including NP/PA, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination.
