Job summary

Heartline Staffing is recruiting a Primary Care Physician to provide preventive primary care for adults 65+, with a focus on Medicare Advantage and chronic disease management.

Physicians see approximately 12–15 patients per day with 20–40 minute visits, in a value-based care model. Training is provided and a collaborative integrated care team supports clinical and administrative needs.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine is REQUIRED.
• Comfortable practicing within a value-based care model.
• Strong communication and team-oriented mindset.

Responsibilities

• Provide primary care for adults 65+.
• Focus on chronic disease management and preventive care.
• Work within an integrated care model and collaborate with support staff.
