Job summary

Heartline Staffing seeks a Primary Care Physician to deliver preventive primary care for adults 65+, with emphasis on Medicare Advantage and chronic disease management. Average 12–15 patients daily, with 20–40 minute visits, in a value-based care setting.

The role includes an integrated care team and on-site clinic support. Training will be provided for value-based workflows and documentation practices.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Value-Based Care experience welcomed but training is provided.
• HEDIS experience is a plus.

Responsibilities

• Provide primary care for adults 65+ within an integrated care model.
• Focus on chronic disease management and preventive care.
• Work collaboratively with NP/PA, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination.
