Job summary

Heartline Staffing seeks a Primary Care Physician to provide preventive primary care for adults 65+, with a focus on Medicare Advantage, chronic disease management, and preventive care. You will see about 12–15 patients per day in 20–40 minute visits within an integrated care team.

Training is provided; there is no RVU pressure. Benefits begin Day 1, with potential relocation support for moves over 50 miles, and collaboration with NP/PA, social workers, and lab/pharmacy teams to coordinate care.

Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine REQUIRED.
• Comfortable practicing within a value-based care model.
• Strong communication and team-oriented mindset.

Responsibilities
