Job summary

Conviva Primary Care, a Humana Inc. subsidiary, is seeking a board-certified or board-eligible physician to provide comprehensive outpatient care for adults and seniors at the Coconut Creek clinic in Florida. Monday–Friday 8:00am–5:00pm; on-site with a collaborative, team-based care model.

Requirements include MD/DO, Florida license, DEA license, and 2+ years in primary or geriatric care. Portuguese and English bilingual.

Qualifications

• MD or DO required; board certification or eligibility in family/internal/geriatric medicine.
• Florida license or ability to obtain; DEA license current or to be obtained.

Responsibilities

• Deliver comprehensive outpatient care to adults and seniors.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Collaborate with interdisciplinary teams to ensure holistic care.
