CenterWell Senior Primary Care in Orlando, FL is seeking a full-time Float Nurse Practitioner or Physician Assistant to join a collaborative team delivering high-quality, value-based care to seniors. The role involves coordinating with physicians, care coaches, and other specialists to manage referrals, hospital coordination, and home health services.

Qualifications

• Master's degree in nursing with board certification, or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in Florida.
• At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Work on-site and contribute to value-based care initiatives.
