Job summary

CenterWell Senior Primary Care in the United States is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative, value-based care team delivering comprehensive care to seniors in an outpatient clinic.

You will work with physicians, pharmacists and care coaches, manage referrals and coordinate hospital/SNF care, participate in on-call rotation, and contribute to growth plans while delivering value-based care.

Qualifications

• Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
• 8+ years of technical experience.
• 2+ years of project leadership experience.
• Willing to participate in TB screening.
• Patient-facing role focused on consumer experiences.
• On-call coverage by phone.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Report to the Area Medical Director.
• This is an in-person job at our outpatient clinic.
