CenterWell Senior Primary Care, part of Humana, is hiring a Nurse Practitioner or Physician Assistant for our Orlando, FL center. You will join a collaborative team delivering comprehensive, value-based care to seniors, coordinating with physicians, pharmacists, and care coaches.

Formación

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in Florida.
• Minimum one year experience in value-based care, primary care or Medicare Advantage settings.
• Minimum one year experience as an APP.

Responsabilidades

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Demonstrate passion for value-based care.
