Job summary

Humana Inc. through CenterWell Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to provide comprehensive, value-based care to seniors. You will work with a multidisciplinary team to deliver high-quality, coordinated services.

Join a center-focused environment with daily huddles, referrals management, and on-call rotation. Requirements include an NP or PA license and at least 1 year of relevant practice, with benefits starting Day 1.

Qualifications

• Master's Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Minimum of 1 year NP or PA experience.

Responsibilities

• Provide comprehensive care to seniors per clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Participate in daily huddles.
• Lead huddles where appropriate (CAs lead).
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and growth initiatives.
• Ensure compliance with licensing and accreditation requirements.
