Job summary

CenterWell Senior Primary Care, a Humana company, seeks a Nurse Practitioner or PA to join our senior-focused care team. You will deliver comprehensive primary care and collaborate with a multidisciplinary team to coordinate referrals, hospital and home health services.

Qualifications

• Master’s degree in nursing or PA program with board certification.
• Active, unrestricted NP/PA license in state of practice.
• Strong communication and interpersonal skills.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Participate in daily huddles and support growth initiatives.
