Responsibilities

• Oversee development and execution of programming and services focused on prevention and management of chronic disease.
• Identify community health needs and develop health programs.
• Coordinate volunteers and work with healthcare organizations, government, and community partners.
• Analyze data, interpret dashboards, and develop targeted interventions and scalable programs.
• Supervise community health workers, youth coordinators and students.
• Contribute to staff sustainability and participate in committees and coalitions.
• Manage health program data, dashboards, and program evaluations to improve quality.
