Responsibilities

• Advance community engagement, referral development, patient access, and program operations.
• Serve as a liaison among patients, caregivers, referring providers, healthcare organizations, community partners, and the interdisciplinary Complex Care team.
• Strengthen awareness of program services and improve access to care for individuals with complex healthcare needs.
• Develop and coordinate community engagement, educational, and outreach initiatives to increase awareness, strengthen referral pathways, and expand access to care throughout Ohio.
• Cultivate relationships with healthcare providers, hospitals, advocacy organizations, community agencies, and institutional partners to promote collaboration and program growth.
• Assist patients and families in navigating complex healthcare systems.
• Support referral and intake processes, verify program eligibility, facilitate patient access and scheduling, and assist individuals transitioning to adult care.
• Collaborate with physicians, nursing, social work, dietitians, administrative staff, and external providers to ensure seamless care coordination and a high-quality patient experience.
