Job summary

The Ohio State University Wexner Medical Center seeks a Population Health Nurse Navigator to join a multidisciplinary team. This role coordinates patient-centered care, education, advocacy, and navigation for patients in value-based care and home-based/post-acute models to improve outcomes and access to services.

It aims to reduce unnecessary hospital utilization by guiding patients through the healthcare continuum, supporting adherence, and linking to community resources.

Qualifications

• BSN required and Ohio RN licensure; CCNE/ACEN accreditation preferred.
• 3+ years of clinical experience; 5+ years in primary care, case management, or post-acute settings desirable.
• Knowledge of evidence-based practice, disease-specific processes, payer protocols, and healthcare continuum.
• Strong clinical judgment, problem-solving, communication, interpersonal, and collaboration skills.
• Ability to work independently; supervisory or preceptor experience is desirable.

Responsibilities

• Coordinate care for complex and high-risk patient populations.
• Facilitate timely follow-up and primary care connections; support transitions of care.
• Connect patients with community resources to address social and non-medical needs.
• Divert preventable inpatient and ED utilization to outpatient, community, and home-based settings.
