Job summary

Qualifications

• Arizona PT license or eligibility as required by state.

Responsibilities

• Evaluate residents to determine needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and function.
• Supervise PT assistants and students in resident care.
• Communicate progress and discharge plans with physicians, families, and team.
• Complete timely documentation in medical records and comply with regulations.
• Provide staff in-services and participate in quality improvement initiatives.

Education
