Job summary

Centene in Texas is seeking an RN for care coordination and case management focusing on pediatric members 0–18 with complex, high-acuity needs. The role supports care across the continuum, develops personalized care plans, educates families, and partners with providers to ensure high-quality, cost-effective services.

Requires nursing degree, Texas RN license (state or compact) and 4–6 years pediatric clinical experience; BSN preferred.

Qualifications

• Graduate from an Accredited School of Nursing with 4–6 years related experience; BSN preferred.
• RN licensure in TX or Compact State; NP licensure if applicable.
• RUG certification must be obtained within 90 days of hire

Responsibilities

• Evaluates the service needs of the most complex/high-risk pediatric members and plans for the best outcomes.
• Develops/assesses long-term care/service plans and coordinates with providers and community resources.
• Coordinates between member/family and care providers to ensure person-centered care.
• Monitors care plans and progress; revises plans to meet member goals.
• Reviews referrals and intake assessments to develop appropriate care plans.
• Educates members and families on disease processes, care gaps, and benefits.
• Acts as liaison between member, physician, and facilities/agencies.
• Complies with policies and state/federal guidelines; travel for assessments required.

Education

Bachelor of Nursing degree (BSN) preferred
