Job summary

Qualifications

• RUG certification within 90 days of hire
• Bachelor's degree in Nursing
• Pediatric experience required

Responsibilities

• Assess and coordinate care for complex/high-acuity members across the continuum
• Develop and monitor long-term care/service plans and identify providers or resources
• Coordinate between members, families/caregivers and care providers to ensure person-centered care
• Educate members and families on disease processes, care options and benefits
• Ensure compliance with state/federal regulations and clinical guidelines
• Act as liaison between members, physicians, and facilities to optimize services
