Job summary

Blue Ridge Cancer Care is seeking an experienced Collaborative Practice Nurse (CPN) to support oncology patients across the continuum of care. The CPN acts as a clinical resource and care coordinator, partnering with providers and nursing teams to improve outcomes and patient experience.

The role focuses on proactive patient management, symptom assessment, education, and navigation of complex needs. Remote and hybrid options are available; travel to outlying clinics may be required based on care

Qualifications

• BSN preferred.
• 5 years' RN experience; oncology experience preferred.
• Oncology Certification (OCN) a plus; Nurse Navigator Certification a plus.

Responsibilities

• Respond to patient inquiries via calls, portal messages, and emails promptly.
• Assess and manage a wide range of symptoms, documenting interventions in medical records.
• Collaborate with physicians via electronic communication, meetings, or calls for urgent cases.
• Coordinate outpatient visits to ensure continuity of care.
• Lead Patient Care Conferences and communicate outcomes to the care team.
• Perform risk stratification and psychosocial assessments, provide emotional support.
• Develop nursing diagnoses and action plans, support oral chemo education and adherence.
• Engage in survivorship and advance care planning visits; make referrals as needed.
• Participate in research, quality, and performance improvement; report navigation metrics.
• Mentor/preceptor to less experienced nurses; identify and address care gaps.
• Stay informed on operations, value-based care, and quality programs; travel to clinics as needed.
