Job summary

Blue Ridge Cancer Care seeks an experienced Collaborative Practice Nurse (CPN) to support oncology patients across the continuum of care. This role focuses on proactive management, care coordination, education, and navigation of complex clinical needs in a remote or hybrid setting.

The CPN acts as a clinical resource, partnering with providers and nursing teams to improve outcomes, enhance patient experience, and support value-based care initiatives.

Qualifications

• BSN-prepared RN; Oncology certification a plus.
• 5+ years RN experience; oncology experience preferred.

Responsibilities

• Respond promptly to patient inquiries via calls, portal messages and emails.
• Use algorithms/pathways to assess and manage symptoms and document interventions clearly.
• Assess urgency of symptoms and manage care to reduce ED visits.
• Coordinate outpatient visits and ensure continuity of care.
• Lead Patient Care Conferences and communicate outcomes to the care team.
• Perform risk assessments and provide psychosocial support; make referrals as needed.
• Develop nursing diagnoses and action plans; educate on oral chemotherapy and adherence.
• Support survivorship and advance care planning; refer as needed.
• Participate in quality and navigation metrics, and advocate for patients.
• Mentor less experienced nurses and identify gaps in care delivery.
