Job summary

Blue Ridge Cancer Care is seeking an experienced Collaborative Practice Nurse (CPN) to support oncology patients across the continuum of care. The CPN acts as a clinical resource and care coordinator, partnering with providers, nursing teams, and interdisciplinary staff to improve outcomes and patient experience.

This role emphasizes proactive patient management, symptom assessment, education, and navigation of complex clinical needs.

Qualifications

• BSN preferred
• Oncology experience preferred
• Nurse Navigator/OCN certifications a plus

Responsibilities

• Respond promptly to patient inquiries via calls, portal messages, and emails.
• Assess and manage patient symptoms and side effects using established pathways.
• Determine urgency of symptoms and coordinate care to prevent ED/hospital utilization.
• Collaborate with physicians for cases needing emergency care or special attention.
• Coordinate outpatient clinic visits to ensure continuity of care.
• Lead patient care conferences and communicate outcomes to the care team.
