Job summary

PACS in Oregon is seeking a Staff Occupational Therapist to evaluate residents, develop treatment plans, and collaborate with physicians, families, and the health team. You will supervise assistants and participate in care conferences.

You will ensure timely documentation, training, infection control, and safety, while supporting fall prevention and ADL independence within the facility.

Qualifications

• Bachelor's degree in Occupational Therapy required or Master/Doctorate preferred.
• Licensure as an Occupational Therapist in the state.

Responsibilities

• Evaluate residents to determine occupational therapy needs and rehab potential.
• Develop, implement, and update individualized treatment plans to improve mobility and functional independence.
• Supervise Occupational Therapy Assistants and training of staff.
• Communicate with physicians, nursing staff, residents, and families regarding progress and discharge planning.
• Maintain compliant documentation and participate in quality assurance initiatives.
