Job summary

Creekwood Nursing and Rehabilitation is seeking an Occupational Therapist to provide patient-centered therapy in a collaborative, uplifting setting. Work with an interdisciplinary team to help residents regain independence, improve safety, and enhance quality of life through individualized treatment plans.

You will supervise assistants, document care, adhere to regulatory guidelines, maintain licensure, and pursue ongoing professional development in a supportive facility.

Qualifications

• Graduate of an accredited occupational therapy program.
• Valid state Occupational Therapist license, where applicable.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Report to the Director of Rehabilitation and oversee OT services.
• Provide screening, evaluation, and treatment planning; discharge planning.
• Ensure compliant documentation and billing per guidelines.
• Supervise OT assistants and rehab technicians.
• Participate in care conferences and interdisciplinary team activities.
• Develop and enhance OT programs to support quality outcomes.
• Maintain environmental safety, infection control, and patient rights policies.
• Maintain licensure and pursue ongoing professional development.

Education

Graduate of an accredited occupational therapy program

Valid state Occupational Therapist license, where applicable
