Excelin Health in Fremont, CA is seeking a licensed Occupational Therapist to join our rehabilitation team. You will implement standards of care for occupational therapy services and collaborate with the Rehab Director to ensure high-quality patient care.

Responsibilities include evaluating patients, developing treatment plans, instructing patients and families, supervising assistants, and coordinating care with other professionals to support safe, independent living.

Qualifications

• OT degree from an accredited program.
• Licensed occupational therapist with active licensure.
• CPR certification required.

Responsabilités

• Evaluate patients’ functional status and home environments to guide care.
• Develop and implement individualized treatment plans and goals.
• Instruct patients and families in therapy techniques and adaptive equipment use.
• Supervise Certified Occupational Therapy Assistants per policy and state regulations.
• Coordinate with physicians and other professionals to ensure a comprehensive plan of care.
• Maintain clinical records and communicate changes to the care team.
