Job summary

Excelin Home Health in Fremont, CA is seeking an Occupational Therapist on a PRN basis. You will assess patients’ functional status, home safety, and needs for adaptive equipment, and develop a plan of care with the physician and care team.

You will implement treatment programs, teach and retrain daily living activities, supervise assistants, document clinical notes, and coordinate with other professionals to ensure integrated patient care.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AOTA/CAAHEA.
• Registered with the National Registration Examination of the AOTA.
• Two years of clinical experience as an occupational therapist.
• CPR certification current.
• Licensed driver with an insured vehicle.

Responsibilities

• Evaluates patient’s functional status (muscle function, endurance, coordination, self care).
• Evaluates home environment for hazards or barriers to independence and identifies equipment needs.
• Develops treatment program and sets goals; communicates plan of care to physician and team.
• May teach new skills or retrain daily activities lost due to illness or injury.
• May fabricate splints and instruct on adaptive equipment use; may train in prosthetic/orthotic devices.
• Instructs family/caregiver and other health care personnel in treatment plan.
• Supervises Certified Occupational Therapy Assistants according to policy and state regulations.
• Instructs and evaluates home health aide care as needed; maintains records.
