Job summary

Excelin Health in Fremont, CA is seeking a dedicated Occupational Therapist to implement standards of care and coordinate with the Rehab Director. You will assess function, plan care, and support patients across home health and clinic settings.

You will evaluate home environments, develop treatment plans, and educate families. Supervision of staff and adherence to service agreements are essential for success.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and the AOTA.
• Registered with the National Registration Examination of the AOTA.
• Two years of clinical experience as an occupational therapist.
• Possesses and maintains current CPR Certification.
• Must be a licensed driver with an automobile that is insured and in accordance with state requirements.

Responsibilities

• Evaluates patient’s functional status and identifies needs.
• Evaluates home environment for hazards and equipment needs.
• Performs work capacity evaluation and refers to vocational training as appropriate.
• Develops treatment program and establishes goals; communicates plan to referring physician.
• Instructs and retrains patient in daily activities and uses of adaptive equipment.
• Fabricates splints and assists with orthotic devices as needed.
• Trains patient in use of prosthetic devices and educates family/caregiver.
• Supervises Certified Occupational Therapy Assistants; adheres to policies.
• Instructs and evaluates home health aide care every two weeks when therapy is sole skilled service.
