Kaiser Permanente is seeking a skilled case manager to provide comprehensive case management for a caseload of low- and medium-risk patients. You will interview patients and caregivers to assess needs, goals, and current services, and develop a patient-centered plan with the healthcare team.

Responsibilities include coordinating resources to ensure continuity and quality of care, obtaining necessary authorizations, and monitoring progress toward treatment milestones while addressing barriers

Formación

• Bachelor's degree in Nursing or related field required.
• Registered Nurse License (California) required at hire.
• CCM certification preferred (if applicable).
