Job summary

Kaiser Permanente Georgia, Atlanta area, seeks a Nurse Case Manager to provide case management for a caseload of low- and medium-risk patients. You will interview patients and caregivers to evaluate needs and goals, determine eligibility and education for admissions, and develop a client-centered care plan with the health-care team.

Responsibilities include coordinating resources, obtaining authorizations, monitoring progress toward care milestones, documenting in medical records, attending case

Qualifications

• Bachelor's degree in Nursing or related field (required).
• Minimum three (3) years of experience in nursing, case management, or a directly related field.
• Georgia RN license required at hire.
