Job summary

Kaiser Permanente seeks an RN for care management across an assigned panel of physicians. You will develop and implement individualized plans, coordinate access to services, and monitor outcomes across disciplines to support safe, cost-effective care.

Collaboration with physicians, discharge planning, and patient education are key aspects of this role. The position requires two years of RN experience in acute or ambulatory settings, with OB/Gyn-specific experience preferred.

Qualifications

• Minimum two (2) years clinical experience as an RN in an acute care or ambulatory care setting required.
• For OB/Gyn positions: two (2) years recent maternal health experience in acute or ambulatory care.
• Demonstrated knowledge of maternal/fetal medicine and high-risk pregnancies.

Responsibilities

• Evaluates and identifies members' needs and develops care management plans.
• Interfaces with physicians and disciplines to create case management programs.
• Monitors plans and revises them to improve outcomes and efficiency.
• Coordinates access to services, authorizations, and transfers as needed.
• Acts as a clinical liaison with outside agencies and supports discharge planning.
