Job summary

Kaiser Permanente in Walnut Creek, CA is seeking a dedicated Case Manager to provide case management services for a limited caseload of low-risk patients. You will coordinate with patients and caregivers to evaluate needs, develop goals, and determine eligibility and education for admissions.

You will enter authorization data, support plan development with the care team, and ensure services align with guidelines. Regular rounds, progress checks, and documentation are expected.

Qualifications

• Bachelor's degree in Nursing or related field; OR minimum three (3) years of experience in case management or directly related field.
• Registered Nurse License is preferred.

Responsibilities

• Pursues effective relationships with others by sharing resources, information, and knowledge with coworkers and members.
• Completes work assignments by applying up-to-date knowledge in subject area to meet deadlines and supports projects with limited guidance.
• Assists in providing services related to the initial case assessment by coordinating with patients and families to evaluate needs, goals, and current services.
• Monitors and evaluates plan of care by coordinating resources and services to assure continuity and quality of care; reviewing authorizations; attending case management rounds.
• Assists in case-planning by partnering in development of a client-focused case management plan with treatment goals.
• Stays updated on current research, policies, and procedures and contributes to system improvements.
• Supports patient disposition by developing comprehensive disposition plans and obtaining authorizations as needed.
• Connects patients with existing services by referring to external/internal resources and coordinating access.
