Job summary

UCLA Health is seeking a self-directed nurse case manager to assess and coordinate care for a diverse group of patients. Qualifications

• BSN or MSN required; ASN accepted for current UCLA Health staff.
• CA RN license and BLS certification required.
• Experience in case management, utilization management and discharge planning.
• Minimum of three years acute hospital experience or equivalent.
• Strong leadership abilities.
• Systems planning and patient care management in a high-volume setting.
• Excellent communication, interpersonal, organizational and analytical skills.
• Ability to work with interdisciplinary teams.
• Knowledge of a large university teaching hospital.
• Pediatric experience preferred.
